What failed

Meetings, tickets, repositories, and decisions each contain only a partial view of the work. A commitment made out loud on Tuesday becomes a half-written ticket on Thursday and a shipped feature that answers a different question entirely. Nobody lied. The intent just decayed as it crossed tools.

What I observed

Every organization I have worked inside had the same failure mode and no instrument for detecting it. Retrospectives catch it late, if at all, and only when someone remembers what was originally promised.

What I am building

Conduit connects those fragments into an evidence graph that can identify lost commitments, context decay, and discrepancies between stated intent and execution.

Current questions

• What counts as sufficient evidence that work was not lost?
• How should Conduit represent uncertainty rather than asserting false precision?
• Which single output creates enough value to support an initial paid audit?

What comes next

Make the thesis falsifiable: pick one organization, one quarter, and one measurable claim about preserved intent, then publish whether it held.
